Drug Overflow: Polypharmacy-related Adverse Drug Reaction Triggered by Hospitalization

A 92-year-old Japanese female nursing home resident was transported to the emergency room owing to fever, cough, and low oxygen saturation. The patient had lived at home with her son and her cognitive and physical function had been relatively high until 4 months earlier, when she developed cellulitis and was hospitalized in another hospital. Her cellulitis resolved with antibiotics administration. She was transferred to a rehabilitation hospital because of declining activities of daily living and decreased food intake.
