Thiamine deficiency after Roux-en-Y gastric bypass surgery is well described in the literature, presenting with ataxia, altered mental status, and eye movement disorders.1 Most present within the first 6 months post-surgery.1 Female patients often present earlier,1 and patients with persistent vomiting postoperatively are more susceptible.2
